Services we provide:

  • assessment and rehabilitation
  • transition from hospital to home
  • wheelchair prescription and modification for conditions like stroke and chronic fatigue.
  • therapy for tamariki with disabilities, and assessments for housing modifications and equipment.
  • anxiety management and group work for adults with severe mental illness
  • support for older adults with serious mental health diagnoses and dementia.

Referral information

You will need a referral to access our services. We accept ACC and non-ACC referrals.

Referrals are accepted from:

  • clinical staff for inpatients
  • healthcare providers
  • external agencies such as NASC, fieldworkers.
